California’s first integrated model where Systems of Care infrastructure meets No Wrong Door service delivery.
Join us in designing and implementing a new model of coordinated care.
Support early-stage development and help scale this work across communities.
A community-rooted, equity-driven respite center for students with disabilities ages 10-22 that integrates healing, inclusive education, family leadership, and economic mobility.
"A transformative, family-led space where students with disabilities thrive through healing, inclusion, and opportunity, and caregivers are supported through respite, connection, and community care."
Bridges to Care is a locally anchored solution, co-designed with Vista-based leaders, VUSEPTA, and families. It responds to a documented regional gap in services for youth with mild to moderate disabilities.
California has over 4.7 million unpaid family caregivers, but only 14% receive adequate respite support. Medi-Cal now funds community-based respite under CalAIM, creating opportunity for sustainable infrastructure.
Vista-Oceanside serves high concentrations of Latino, Filipino, and military families caring for youth with disabilities. Caregivers repot burnout, isolation, difficulty navigating Medi-Cal and Regional Center systems, and limited culturally responsive options.
Bridges to Care activates regional talent and caregiving leadership through deep co-design with impacted stakeholders, integration of disability justice values, and direct alignment with Vista's Community Schools and wellness priorities.

The Bridges to Care Respite Center is being designed to support students with disabilities (ages 10–23) and their caregivers. Rooted in equity and community leadership, the center will provide a safe, inclusive space where families can find respite, connection, and opportunity.
For Families: Walk in for respite care. Leave with connections to housing, healthcare, education supports, and economic assistance - all coordinated through one integrated hub.
For Systems: Schools, health plans, CBOs, and county agencies can refer families knowing they'll received coordinated care with shared data, warm handoffs, and follow-up.
